12 Monthly Prizes are available in the entire T.J. Maxx Customer Satisfaction Survey Sweepstakes. Each prize consists of a $500 T.J. Maxx Gift Card. The total prize pool is $6000.

The T.J. Maxx Customer Satisfaction Sweepstakes participants must abide by the contest rules and requirements to be eligible for the draw. Like they must
- Be a resident of the 50 United States including the District of Columbia and Puerto Rico.
- Be at least 18 years of age or the age of majority in their respective states of permanent residence as of the sweepstakes entry date.
- Not be any of the employees, officers, and representative of T.J. Maxx, the survey administrator, and the related companies or individuals and their immediate family or household members.
- Visit a participating T.J. Maxx store and receive a cash register receipt with a survey invitation and number printed on it
- Complete the survey on time to access the Online Survey and enter the applicable monthly drawing.
T.J. Maxx Customer Satisfaction Survey Sweepstakes Online Entry:
In two ways you can take part in the T.J. Maxx Customer Satisfaction Sweepstakes- through Online Survey and by Write-In request by mail. If you are an eligible individual and have a survey invitation on a recent T.J. Maxx receipt, online is the best way to participate. To do it, you can
- Visit the T.J. Maxx Customer Satisfaction Survey website on your survey invitation receipt or log on to www.tjmaxxfeedback.com
- Enter the Survey Number, Select the Date and Time located towards the bottom of your T.J. Maxx receipt in their respective boxes. (Look at the example image on the right-hand side of the page if you can’t find the information on your receipt).
- Click on the “Start” button and answer the T.J. Maxx Customer Satisfaction Survey questions to the best of your knowledge.
- Submit the survey and enter the requested information to complete the sweepstakes registration form.
Once you are done, you will be automatically entered for a chance to win the monthly customer satisfaction sweepstakes.
T.J. Maxx Customer Satisfaction Sweepstakes Mail-In Entry:
Alternatively, you can participate in the T.J. Maxx Monthly Sweepstakes by mail without any survey completion. For the mail-in entry, you have to take a 3”×5” card or paper. Legibly write your full name, complete mailing address, phone number, and e-mail address on it. Then, place the card in a #10 envelope with proper postage affixed and mail it to the following address.

To be eligible for the corresponding drawing entry, make sure that your mail-in entry is complete and postmarked by the final day of the month. It is also needed to be received by the fifth day of the next month.
Important Information about T.J. Maxx Survey and Sweepstakes:
- Regardless of entry method, the limit is one entry per household each month. Entries will not be carried forward.
- One random drawing will be held on or about the 10th day of each month to select the Monthly Prize Winner.
- After drawing, the winner will be notified by telephone, email, USPS mail, or other home delivery service of choice.
- For eligibility confirmation to claim the prize, a winner must sign and return the affidavit with necessary documents within 5 days of the initial notification.
- If any prize winner cannot be reached within 3 days of the first attempt of the notification, an alternate winner may be selected for the prize at issue.
